Re: Request for Reassessment — [Name of Person Receiving Care, Date of Birth]

Dear [Name of Care Coordinator / Home Care Team],

I am writing to formally request a reassessment for [full name], who is currently receiving home care services through [CLSC name].

Since the last assessment on [approximate date], there have been significant changes in [his/her/their] situation that I believe are not reflected in the current care plan. Specifically:

  • [Brief description of change 1 — e.g., "She can no longer bathe independently and requires full assistance."]
  • [Brief description of change 2 — e.g., "There have been three falls in the past two months, including one that required a hospital visit."]
  • [Brief description of change 3 — e.g., "She was recently diagnosed with [condition], which has significantly affected her mobility."]

[Optional: describe change in caregiver situation — e.g., "I have also had to reduce my own work hours significantly to provide additional care, and I am concerned about my ability to continue at this level without additional support."]

I am requesting a home visit at your earliest convenience to review the current care plan and discuss whether additional services are appropriate.

Please confirm receipt of this request and let me know the expected timeline.

Thank you for your attention to this matter.

Sincerely,
[Your name]
[Date]

Notes on tone

Keep the letter factual and respectful. This is not the place to express frustration — that is better addressed if the response is unsatisfactory and you escalate to a formal complaint. A clear, professional tone tends to get a faster and more cooperative response.
